When aging gets complicated — medically, legally, financially, emotionally — a care manager sees the whole picture and builds the plan. You get a knowledgeable guide beside you for the entire journey.

Most families call us at a moment like one of these.

  • A parent fell, or landed in the ER, and suddenly needs more help than anyone planned for.

  • The dementia has been steadily increasing — the missed bills, the confusion, the drive that ended somewhere it shouldn't have — and it's clearly time to act.

  • A hospital is ready to discharge your loved one, and they need more care at home than your family can realistically provide.

  • You live three states (or an ocean) away, and you need trusted eyes on the ground.

If any of that sounds familiar: you're not behind, and you're not alone. This is exactly the work we do — and we've been there with our own families, too.

Crisis & Remote Support

Always ready to step in when you need it most.

Whether you are managing a crisis from across the country or coordinating a sudden hospital discharge locally, our team provides immediate support, stability, medical advocacy, and hands-on guidance.

We handle urgent physician calls, organize immediate care assistance, and establish a long-term care roadmap so your family can pause, recharge, and breathe.

Understanding Care Management

First, the question almost everyone asks.

Most people have never heard the term “care management,” or they assume it's the same as the caregivers who come to the home. It isn't — and knowing the difference is the first step to getting the right help.

Care Management

The expert strategic layer

Sees the entire picture — medical, legal, financial, and emotional. Assesses the situation, builds a plan, coordinates doctors, steps in during crises, and adapts as needs change.

Home Care

Hands-on daily care

Sends trained, compassionate caregivers into the home for day-to-day support: personal care, companionship, meals, housekeeping, and transportation.

Care management is a holistic, client-centered approach to supporting older adults through the challenges of aging. It includes:

Assessment and ongoing monitoring

Family caregiver coaching

Education and advocacy

Crisis intervention

Navigating healthcare, housing, legal, and financial decisions

Care planning and problem-solving

Self-Assessment

We're a good fit if…

If the person you love…

  • Has multiple medical or psychological issues to manage at once

  • Can no longer live safely in their current home

  • Isn't getting the care they need and needs someone to advocate for them

  • Is confused about their own financial or legal situation

  • Has limited family nearby — or none

…or if your family…

  • Has realized something is wrong, but doesn't know what challenges even exist

  • Is burned out, or at odds over what to do next

  • Doesn't have the time, expertise, or proximity to manage chronic care needs

  • Needs guidance handling the behaviors that come with dementia

If you recognize your situation in that list, a conversation with a care manager is the right next step.

Care Management Services

What a care manager does for your family.

Healthcare Advocacy

We represent and support your loved one's interests inside a healthcare system that's hard to navigate alone — so they get the care, attention, and resources they actually need.

Coordination of Care

We keep everyone rowing in the same direction: doctors, specialists, providers, and family, in communication and aligned — for better care and better outcomes.

Life Transitions

We conduct comprehensive in-home assessments to support big changes — a move to a new living situation, a step up to a higher level of care, or new services added to preserve independence at home.

Crisis Intervention

When an emergency hits, we help your family navigate the ER, hospitalizations, and rehab stays, and make sure adequate care is in place. For families at a distance, we're the emergency contact who's always there.

Legal & Financial Coordination

We work alongside your legal and financial professionals — attorneys, advisors, trust officers — so the practical side of aging is handled with a plan, not a scramble.

“Where do I start?” — Patrick's Story

Patrick lives in Europe and could only visit his mother now and then. Between visits, the small changes added up — an empty fridge, a slipping routine, and finally, getting lost on a road she'd driven for years. He was certain she was in cognitive decline, but had no idea where to begin.

He called AffirmiCare. Together with a care manager, Patrick built a road map to secure his mother's safety, arrange medical support, and direct care in her home. He returned to his life abroad knowing a plan was in place, a guide was a phone call away, and his mom was truly cared for.
